If we scheduled you for a first-time appointment at Footprints Counseling, please log into the Client Portal and complete the documents. PDF copies of the forms are below. These will need to be completed and signed by the client or the parents or guardians of the clients prior to the first session.
Pay a bill
Do you have a copay or a bill you want to pay off. We have several options. You can send a ck in by snail mail or conveniently pay by PayPal or Square. When you are a client you can also save your credit/debit info on file and we can charge the card on file.
