If you have been scheduled for a first time appointment at Footprints Counseling please click on the documents below. Please download for your records and bring in the signed signature pages to your 1st intake session. These will need to be signed by the client or the by the parents or guardian of the clients.
Pay a bill
Do you have a copay or a bill you want to pay off. We have several options. You can send a ck in by snail mail or conveniently pay by PayPal or Square. When you are a client you can also save your credit/debit info on file and we can charge the card on file.
